Horse Racing - does API indicate if horse/runner is supplemented

does API indicate which horses are supplemented. I have been looking on the metadata for such information.
Also exactly what do the following mean
ADJUSTED_RATING
FORECASTPRICE_NUMERATOR/FORECASTPRICE_DENOMINATOR
OFFICIAL_RATING
FORM - which is quite often a one or 2 digit number so its not the placing over the last few races?
